AGREEMENT OF RELEASE AND WAIVER OF LIABILITY & ACCEPTANCE OF RISK.
All customers and visitors to Cycle Collective Studio are subject to Cycle Collectives Indoor Cycling Studios Terms and Conditions, Assumption of Risk, Liability Waiver and Release. By sighting and agreeing to this, you agree to the Terms of Service set forth below. If you do not agree to these terms, please do not use and/or participate in classes, events, courses, work-shops and using the exercise equipment and facilities provided by Cycle Collective
As a participant in Cycle Collective fitness activities (classes, events, workshops courses), I recognize and acknowledge that there are certain risks of physical injury including, but not limited (1) minor injuries such as abrasions, contusions, and sprains; (2) major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks, and concussions; and (3) catastrophic injuries including paralysis and I have no physical condition which would present a risk of injury to me through my participation in Cycle Collective fitness activities (classes, events, courses, workshops).
Notwithstanding any instruction or consultation by a representative of the Cycle Collective, I agree to assume responsibility for any such injuries, damages or loss which I may sustain as a result of participating in any and all activities connected with or associated with Cycle Collective except if caused by the sole negligence of the Cycle Collective. I hereby release, waive and discharge the Cycle Collective, its trainers/ coaches, officers, agents or employees from any and all liability, claim, damages and losses arising out of any loss, damage or injury that may be sustained by me or to any property belonging to me while participating in Cycle Collective fitness activities (classes, workshops, courses, workshops).
It is my express intent that this Agreement of Release and Waiver of Liability & Acceptance of Risk Agreement shall bind the members of my family, my heirs and assigns.
I further agree that participation in any fitness activity (classes, workshops, courses, events) will be at my own discretion and judgment. I further understand that should the Cycle Collective discover that I have not satisfied any one of these requirements, it may, but is not required to, terminate my participation and/or contract.
I am 18 years of age or older. I have read and fully understand the above Agreement of Release and Waiver of Liability & Acceptance of Risk.
